  • Medicare

Please verify your insurance benefits before your appointment to ensure we are in-network. If you are uncertain about your coverage, feel free to contact our office for assistance. We recommend checking with your insurance provider to confirm your benefits, as each plan may have different co-pays, deductibles, and coverage policies.

Insurance Verification:

  • It is your responsibility to provide us with accurate and up-to-date insurance information. If your insurance changes, please notify us prior to your appointment.
  • We will submit your insurance claims on your behalf; however, any remaining balance after your insurance has processed will be your responsibility.
  • For non-covered services or out-of-network visits, payment is due at the time of service.

Co-pays and Deductibles:

  • Co-pays, co-insurance, and deductibles are due at the time of service. These amounts will be determined by your insurance provider, and we will inform you of your financial responsibility before your visit when possible.
  • If you are unsure about your co-payment amount or have questions about your deductible, please contact your insurance company directly.

Self-Pay Patients:

If you do not have insurance or choose not to use your insurance, we offer self-pay rates. Please contact our office for pricing and payment plan options.

Billing and Statements:

  • After your appointment, we will process your insurance claim and send you a statement for any remaining balance.
  • Statements will be sent monthly for any outstanding balance. Please pay your balance promptly to avoid any additional fees.
