
At Omega OB/GYN, we are committed to your privacy and the security of your health information. We understand that there may be times when you need to access your medical records for a variety of reasons, such as transferring care or reviewing past treatments. Below, we provide the necessary information to request your records quickly and efficiently.
How to Request Your Medical Records:
Online Request:
To request your medical records, please complete our online form (click here) with the required details. This is the most efficient way to submit a request.
In-Person or Phone Request:
You can also request your records by visiting our office or calling our staff at (817) 468-3255. Please provide the following information:
- Full Name
- Date of Birth
- Specific documents or dates of treatment needed
- Your contact information
Third-Party Requests:
If you would like a third party, such as another healthcare provider, to receive your records, please complete an authorization form (available upon request). This ensures we have your consent to release your information.
Processing Time:
Medical record requests typically take 5-7 business days to process. We will notify you once your records are ready for pick-up or if any additional information is needed.
FMLA Paperwork & Short Term Disability:
If you need FMLA (Family and Medical Leave Act) paperwork processed, please note that there is a $25 processing fee for this service. This fee covers the administrative time required to accurately complete and review the forms. Payment is required at the time of submission. - Short Term Disability: Short-Term Disability must be approved by your doctor. Once this is approved forms can be filled out and sent to the insurance company. For pregnancy – Vaginal birth is 6 weeks and for C-Section it is 8 weeks. This time starts the day you give birth. Your doctor’s approval must be made if disability is taken before delivery. You will have to have a medical diagnosis regarding disability leave before delivery and this must be approved by your doctor. If you take time off for disability, and this was not approved by your doctor, then this is between you and your job. For surgery, your doctor will let you know the amount of time needed for recovery and this will be put on the form along with the reason you will be out for disability.
- FMLA: For FMLA you have up to 12 weeks of unpaid leave for pregnancy, surgery or other medical reasons. FMLA secures your job but does not pay while you are out. FMLA secures your job as long as you don’t exceed 12 weeks per year. This may be taken all at once or intermittently. For example, if you choose to take 2 weeks off prior to delivery, then once you deliver, you have 10 weeks for postpartum recovery. For surgery, this applies as well. If you want to take intermittent time off, then this time will also be deducted from the overall 12 weeks. Both intermittent and consistent medical leave will need to be approved by the doctor so your company will know how much time you will take for this. Patient Rights:
You have the right to access, review, and request a copy of your medical records at any time. If you believe any information is incorrect or incomplete, you may request amendments to your records as allowed under HIPAA regulations.
