FMLA Paperwork Processing Fee

If you need FMLA (Family and Medical Leave Act) paperwork processed, please note:
✔ There is a $25 processing fee for this service.
✔ This fee covers the administrative time required to complete and review forms.
✔ Payment is required at the time of submission.

Short-Term Disability

Short-Term Disability must be approved by your doctor before forms can be completed and submitted to your insurance company.

Pregnancy & Maternity Leave

  • Vaginal Birth: 6 weeks of disability leave
  • Cesarean Section (C-Section): 8 weeks of disability leave
  • Start Date: The leave begins on the day you give birth
  • Before Delivery: If you need disability leave before giving birth, your doctor must approve it based on a medical diagnosis.

If your disability leave is not approved by your doctor, this is between you and your employer.

Surgical Recovery

  • Your doctor will determine the necessary recovery time based on your surgery.
  • This time will be documented on your disability form, along with the medical reason for leave.

Family and Medical Leave Act (FMLA)

FMLA provides up to 12 weeks of unpaid leave for pregnancy, surgery, or other medical reasons.

Key FMLA Benefits & Guidelines

✔ Job Protection: FMLA secures your job during your leave (up to 12 weeks per year).
✔ Unpaid Leave: FMLA does not provide income while you are out.
✔ Flexible Leave Options:

  • You may take all 12 weeks at once or use it intermittently.
  • Example: If you take 2 weeks off before delivery, you have 10 weeks remaining for postpartum recovery.
  • Surgical Leave: The same applies if you need time off for surgery.
    ✔ Doctor’s Approval Required: Your doctor must approve both intermittent and continuous medical leave for your employer’s documentation.
